Payment for Services
Payment for services is due before or at the time you receive them, minus the deposit you paid when you scheduled your appointment.
We accept payment by cash, check, debit card and most major credit cards.
We also offer third-party financing through Cherry and CareCredit. If you’re interested in this financing, we’ll give you a treatment plan after your first appointment that the financing company can use to provide options to break your payments into manageable monthly amounts.
If you’d like to make arrangements for financing before your appointment, please call us one week before your visit so we can help you.
Note that you will be responsible for paying for your new patient appointment.
Appointment Deposits
When you make an appointment, we’re reserving time just for you. To ensure our team is fully prepared to support your unique needs, we ask for a deposit when booking.
- $200 deposit for a new patient appointment
- 20% for appointments scheduled under 2 hours
- 50% for appointments scheduled over 2 hours
This deposit is fully credited toward your visit and lets us provide the thorough, personalized care that sets our practice apart.
Missed Appointment Policy
We take great care in preparing for your appointments, reserving time exclusively for you. As with a hotel room, if you don’t show up for your reservation, the room sits empty. Missed appointments create challenges not only for our team but also for patients who are eager to start treatment and are waiting for an appointment.
Because of this, we ask that you give us at least 2 business days’ (M-Th) notice if you need to reschedule or cancel your appointment. If you miss your appointment or don’t provide enough notice, you’ll lose your deposit and will have to put down a new deposit to reschedule.
We understand that emergencies and illnesses do arise, and we will make exceptions accordingly. Help us serve you and all patients better by keeping your appointments and communicating any scheduling conflicts ahead of time.
Dental Insurance
We focus on your health, not what insurance may cover – which is less and less every year. Dental insurance doesn’t cover the high quality materials we use at Sage Dental Wellness, nor laser dentistry or many of the other biological and holistic services we offer. Because of this, we are a fee-for service provider and are not under contract with any dental insurance companies.
That said, if you have out-of-network benefits, you CAN use your dental insurance in our office.
- Before scheduling with us, contact your dental insurance provider and ask if you have out-of-network benefits. Some out-of-network benefits cover the same amount as in-network; some cover less. Each insurance plan is different. We have no control over what your plan will pay.
- Ask your insurance company if there’s a difference in coverage for out-of-network versus in-network and understand what that difference is.
- Be prepared to pay for all dental expenses the day of service and to be reimbursed later by your insurance provider based on your out-of-network coverage.
- At your new patient appointment, we’ll give you a personalized treatment plan with dental CDT codes for recommended treatment. You can review those codes with your insurance company to see what your plan will cover. This is called a pre-authorization and will help you estimate your out-of-pocket expense before treatment. Please talk with us if we can help you maximize your dental benefits before we begin treatment.
- At each appointment, payment is due in full. We’ll give you a “superbill,” which is an itemized receipt for what you paid, along with a list of completed treatment on an ADA claim form for you to submit to your insurance company. This tells them what was done, what you paid and how much, if anything, they need to reimburse.
Please know that many of our services have the same dental codes that insurance companies will recognize. However, our practice isn’t like a traditional one and doesn’t fit into the typical dental insurance model. We provide you with the highest level of care and offer additional services that are often not reimbursed by insurance
Returned Check Policy
If your check to us is sent back due to insufficient funds, you will be charged $38 plus a rebilling fee. Payment is expected as soon as you’re notified of the returned check. If we don’t hear from you within 48 hours, we’ll charge your credit card on file or submit for payment to cover the outstanding balance.
Outstanding Balance Policy
Under rare circumstances, if you have a balance due, you’ll receive an invoice from us via mail, email, or text, and your balance is due upon receipt.
To ensure that we can continue providing timely, quality care to all our patients, balances not paid promptly will be subject to accruing late fees. If a balance is past due by 30 days, a 4% finance charge will also be added each month past due, and we won’t be able to schedule any additional treatment until the balance has been paid.
